Colibri, a platform result of Nextage’s successful experience in Decipher EU PCP, designed for chronic patients, focused on Diabetes, but scalable to different scenarios such as those described by HSMonitor. The platform is modular (allowing integration of vertical solutions, e.g. AI tools, communication with wearable devices, communication and training plugins), multi language (currently in english, italian and french, but new languages can be easily included) and is able to interface with other systems (e.g via REST API), creating direct integration with the external sources to import/export data by defining and using automated procedures and international standards.
